The Home Visiting Project has
run successfully since January 1996. It provided in-depth advice,
information and advocacy to people who were housebound and generally not able to
use existing services due to ill-health disability or home caring
responsibilities. It was funded jointly by Hillingdon Primary Care Trust and the
Local Authority. The Home Visiting Project was a referral only service, located in the Uxbridge office. Referrals to the Home Visiting
Service were received from Social Services, Primary Health Care Teams and
advisors working in each of the three offices of HECA. Most
of the clients seen have complex multiple problems and each home visit takes an
average of two hours and generates a lot of time consuming follow-up work Clients whose sole income was
from means tested benefits, such as Income Support (frequently Income Support
supplementing a State Retirement Pension), have an extremely restricted
lifestyle and are often unable to afford nutritious, fresh food or sufficient
heating for their homes. Maximising income by obtaining
awards of Disability/Welfare Benefits has a significant impact, enabling elderly
clients and those suffering from chronic illness or disability to maintain
independent lives in their own homes, thus reducing the detrimental effects of
poverty on their overall health and well-being. The
Home Visiting Service provided equitable access to a full Citizens
Advice Bureau service for those who were housebound.
Many people in this sector of the Community are amongst the most
financially and environmentally deprived, and the advice and help received from
the Home Visiting Service enabled them to improve their lifestyles and overall
health and well-being. For more information: click below to view or right click below and Save Target As... to download the Home Visiting Project Six Monthly Report.
