The GP practice is near Uxbridge shopping centre and is readily accessible by public transport.

HAREFIELD HEALTH CENTRE


Harefield Health Centre is the only GP Practice located in the village. It is a particularly appropriate location for a CAB Outreach Service because it is a semi-rural village and the residents do not have easy access to any of the Hillingdon CAB offices.

GP Surgeries Project aims are to:-

  • Reduce GP workload by provision of a source of help and advice to deal with patient’s problems which are non-clinical and to which the GP often cannot provide a solution.
  • Improve the overall health and well-being of patient’s by giving access to a Citizens Advice Bureau Service in health centres and GP Surgeries.
  • Provide a holistic approach to health care providing help and advice to relieve the stress and worry of the practical side of life problems such as bereavement and relationship breakdown and to reduce economic and environmental deprivation caused by debt, low income, inadequate housing etc.

Case Study:-
A white, female, 73 years old, who has physical and mental health problems living alone in Local Authority accommodation, attended the GP outreach advice service at Harefield Health Centre.  Her only income was from Pension Credit of £124 per week and she had just received a fuel bill for £365 which she could not afford to pay.

                 

Our Adviser identified that the pensioner met the qualifying criteria for Attendance Allowance (AA) and an increase in her Pension Credit (PC).  She assisted her to complete a claim and as a result AA was awarded at the higher rate of £70.35 per week totalling £3,658 per year and she received a backdated payment of £482.

 She was also awarded the Severe Disability Premium on her PC which gave her an extra £52.85 per week totalling £2,748 per year and a backdated payment of £846. 

 Our Adviser also assisted the patient with an application for a grant from the British Gas Energy Trust to help with payment of the client’s fuel bill.  She received a grant of £365 which cleared her outstanding gas bill.
